Etiology-Dependent Treatment Responses in Amblyopia: The Role of Cortical Suppression and Neural Connectivity

Abstract

Amblyopia is a neurodevelopmental visual disorder caused by abnormal visual input during critical periods of cortical development. Most research evaluates the effectiveness of treatments without properly distinguishing between the neural mechanisms associated with different etiologies. Accordingly, the purpose of this study was to analyze how the etiology of Amblyopia influences responsiveness to conventional and modern treatments through evaluation of varying measures of cortical connectivity, suppression, etc. This study utilizes a secondary research design via analysis of peer-reviewed primary and secondary scientific literature, including clinical studies, neuroimaging research, and quantitative treatment outcome data, as well as interpretation of possible effective treatments based on mechanism and characteristic attributes of each etiology. Results found that strabismic amblyopia generally involved strong cortical inhibition and interocular suppression, warranting binocular therapy as the most plausible treatment. Anisometropic amblyopia was found to involve weakened cortical connectivity, supporting perceptual learning and refractive correction as the most appropriate interventions. Mixed-mechanism amblyopia exhibited overlapping abnormalities involving both suppression and reduced connectivity, indicating that individualized multimodal treatment approaches are necessary. The findings suggest that efficacy is strongly dependent on etiology-specific neural mechanisms. This study supports the development of individualized treatment strategies that target the specific cortical dysfunction associated with each subtype.
